Feedback helps improve support
You can give a compliment, suggestion or complaint. You may involve a trusted person or advocate, and you can choose not to provide your name.
- We listen and acknowledge your concern.
- We explain the next steps and expected communication.
- We aim to respond fairly and without retaliation.
- We record outcomes and use feedback to improve quality.
Immediate safety: Call 000 if someone is in immediate danger. For urgent but non-emergency service concerns, call Medease directly rather than relying only on this form.
Other complaint pathways
You may seek help from an independent advocate or contact the NDIS Quality and Safeguards Commission. You do not have to complete Medease’s internal process first.
